Documents required for Senior citizen card

1. Fill out form in triplicate

2. Any one address proof (Ration card; Valid passport; Election card; Electricity bill or Telephone bill).

3. Any one age proof (Valid passport; PAN card; School leaving certificate or Age proof certificate from a Government Hospital).

4. Any one Birth proof (have to above 60 years old)

5. In case of name change for women, submit marriage certificate

6. 4 stamp size photographs (Attach two photographs on the white form and two on the pink form)

7. Letter from doctor showing ailments if any, medication in use/ allergic to.

8. Report from pathologist showing blood group.

9. Original documents should be brought for verification.

It will take approximately 60 days to 90 days from the day of submission of application for getting your ID.

Apply In personally in collector office:

1.If you want to apply for the senior citizen certificate, please approach the concerned Sarpanch/Jan Seva Kendra/Talati/Mamlatdar office etc.

2.You have to go to any of the above authority office to get details and procedur for applying and need to get the application form.

3.You have to make sure that you have all the proper documentation mentioned below in the “Required documents” list.

4.After getting form you have to submit the completed form signed along with the documents (self attested or true copy) to the respective authority in your district.

5.Your application will be processed by the government employee and verification of submitted documents will also be carried out by them.

6.After successful completion of the process your senior citizen certificate will be delivered to the applicant through Indian speed post or personally as per the applicable norms according to state .

7.The total process of issuing the Senior Citizen Certificate to individuals will take around 40 working days.

Through Common Service Center

1.You have to go to your nearest CSC in your area.

2.You will get Senior citizen form from the authorities and they may charge nominal amount for form may be 5rs or 10rs.

3.After getting Senior citizen form you have to submit the completely filled application along with required documents(self attested or true copy) which are mention below. You may be told to pay fees for processing (nominal).

4.After submitting form and documents CSC Operator will verify the details and documents provided by you and start to process the application online.

5.After processing applicant will get the Reference ID for the application made by you. Refrence id is unique for every process the CSC make you have to keep it safe for future use.

6.After getting Reference id your application will be processed by the respective department of a state as mention above in personally apply.

7.After processing of application the applicant will get Sms notification about the status of certificate on his registered mobile number which you have provided during application.

8.After you received confirmation of your application you can get the hard copy of the certificate from where you applied (CSC) upon notification.

You can even apply Online

1. After going to government website find the tab and select “Senior Citizen Certificate” , go through all the details given there and click on the apply online button given at the end.

2. In the opened page, individual have to select login option email or mobile from the drop down menu for login.

3. Completely fill the form given on the page prompt to login by clicking login button in the bottom.

4. If you are a new citizen to this government portal, then click on “Click For New Registration (Citizen)” link shown on the page, it will take you to user registration page to register you as a citizen.

5. Once you created login id and password, you have to select “Request a New Service” button on top right corner of the government website page.

6. Select the Senior citizen certificate service to reach the new page.

7. You have to press the respective button and complete the other page which prompts and there you have to upload required documents to apply online by clicking submit button.(note: only sacn copy of documents will upload no photograph will upload)

8. After your successful submission of application you have take the printout of your application form and proceed further with the online payment option.

9. You can pay using any of the two options available in government portal.

10. You can use E-Wallet or.

You can use other payment Gateways.

11. After successful payment your application will be processed by the concerned department.

12.bYou will get SMS and Email for the status of your application.

13. Once you complete the application process, you have to use the application number to get the certificate online by using download issued document option in government portal.
